Comprehensive Men’s Health & Hormone Consultation

Investment: $247

A comprehensive evaluation for men experiencing concerns involving energy, sleep, focus, mood, body composition, sexual health, metabolic health, or possible hormonal changes.

Your consultation includes:

  • Comprehensive symptom, medical-history, and lifestyle review

  • Review of current medications and health risks

  • Evaluation of hormone, metabolic, cardiovascular, sleep, and sexual-health factors

  • Review of available laboratory results

  • Individualized treatment and wellness recommendations

  • First month of testosterone or enclomiphene medication when prescribed and clinically appropriate

  • Initial treatment instructions and education

The consultation fee covers the evaluation and personalized treatment plan regardless of whether medication is ultimately prescribed.​ Required laboratory testing is separate. 

Testosterone Therapy Transfer of Care

Investment: $175

For men who are currently receiving testosterone therapy through another provider and would like to transfer their ongoing treatment to Prime Optimal Health.

Your transfer consultation includes:

  • Comprehensive review of your current testosterone regimen

  • Review of treatment response, symptoms, and side effects

  • Review of recent laboratory results and prior treatment records

  • Evaluation of current dosing and monitoring needs

  • Individualized recommendations for continued therapy

  • Initial testosterone medication order when continued treatment is clinically appropriate

  • Transition into the ongoing TRT program

Recent, clinically appropriate laboratory results are required. Additional testing may be necessary before a prescription can be continued and is billed separately. Following the transfer consultation, ongoing testosterone therapy is $150 per month.

Ongoing Men's Hormone Therapy

Testosterone Replacement Therapy: $150/month

Enclomiphene Therapy: $125/month

Ongoing therapy is available for established patients when clinically appropriate. Your treatment option will be selected based on your symptoms, laboratory results, health history, fertility goals, and personal preferences.

Both programs include:

  • Prescribed testosterone or enclomiphene medication

  • Routine treatment-related follow-up appointments

  • Medication management and routine dose adjustments

  • Treatment-related portal messaging

  • Ongoing prescription coordination

Laboratory testing is required before starting treatment and at clinically appropriate intervals during ongoing therapy for safety and treatment monitoring. Laboratory testing is not included in the monthly program fee and may be completed using insurance or available self-pay options.

Routine monitoring appointments related specifically to testosterone or enclomiphene therapy are included. Appointments involving new, unrelated, multiple, or more complex concerns fall outside the scope of the hormone therapy program and are billed separately.

Extended follow-up appointments are available to active program participants at the discounted rate of $125.
